About Derek
I'm Derek Kaneshiro, a principal software engineer in Hillsboro, Oregon. For ten years I owned the HIPAA-regulated platform behind Zimmer Biomet's orthopedic recovery products — 500,000+ patients, PHI in every request, accountable for its reliability throughout.
I built the rules-driven engine that decides what each patient receives on each day of recovery; it became core product IP and still serves them. I replaced scattered per-endpoint permission checks with one centralized, auditable authorization model, because an auditor was going to ask who could write what and the architecture had to answer. And I led the R&D that prototyped Claude and Semantic Kernel integrations for that environment, taken through Architecture, Cloud, Security and risk governance.
I also build the AI agent infrastructure my own development runs on: narrow composable sub-agents, MCP tool surfaces, and multi-provider routing selected per workload on accuracy, latency and cost — plus an evaluation framework I open-sourced, because AI systems degrade quietly and “it seems better” is not a measurement.
Earlier I directed 30 engineers building the practice-management system for 750+ veterinary hospitals.
